What is Form CR-01 and Form CR-02?
Two forms, two applicant types. Picking the right one is your first step.
Form CR-01 is the official PAN correction form for individual applicants — salaried persons, self-employed individuals, Hindu Undivided Families (HUFs), and minors. It replaced the older Form 49A correction request from April 1, 2026. If you're an individual fixing any detail on your PAN, this is your form.
Form CR-02 is the equivalent for non-individual entities — companies, partnership firms, LLPs, trusts, societies, AOPs, BOIs, and government bodies. It replaced the previous Form 49B correction format from April 1, 2026.

Before you fill the form: your preparation checklist
Most form-filling problems start before you write a single letter. You sit down, realise you're missing a document, and half-fill the form from memory. That's how errors creep in.
Get everything ready first. Here's your checklist.
Your Aadhaar card
This is your reference point for the name. Whatever you write in the name field must match your Aadhaar exactly — same spelling, same order, same spacing.
Your existing PAN card (or its number)
You'll quote your current PAN on the form. Lost the card? Keep a copy of the FIR, as it may be required in place of the card.
Supporting documents

Two passport-size colour photographs
Recent, white background, size 4.5 cm × 3.5 cm. You'll need these for CR-01.
Your signature
Practise a clean signature that fits inside a small box without crossing the lines. This matters more than you'd think.
An active email ID
All updates and your acknowledgement land here. Use one you actually check.
A working mobile number
Preferably your Aadhaar-linked number, since OTP verification runs through it for the online route.
Field-by-field: how to fill every part of the form
This is where corrections succeed or fail. Each field has a right way and a common wrong way. Go through them one at a time, and match your entries to your supporting documents exactly.
Remember the golden rules that apply to every field: black ink, block letters, one character per box, and one blank box between words. Tick the box in the left margin only for the fields you are actually correcting.
Name (and name as per Aadhaar)
What to enter: Your full name in block letters, one letter per box. The form has a name field and a separate "name as per Aadhaar" field. Your name must match your Aadhaar exactly, because the Aadhaar name is what gets printed on the PAN card.
Example: If your Aadhaar reads "SURESH SARDA," write it exactly that way — first name, then surname, with a blank box between them.
Common mistake: Adding a title like Shri, Mr., Smt., or Dr. Titles are not allowed anywhere in the name field.
Expert tip: If your Aadhaar shows only initials (like "M.S. Kandaswamy"), write the expanded full name in the main name field — "MADURA SOMASUNDRAM KANDASWAMY." Never use abbreviations in the main name column.
Father's name
What to enter: Your father's full name, split as first name, middle name, and last name.
Example: RAJESH — KUMAR — SHARMA, each in its own set of boxes.
Common mistake: A married woman writing her husband's name here. The form asks for the father's name, not the husband's.
Expert tip: Even if you plan to print your mother's name on the card, fill the father's name accurately too — the field is mandatory.
Mother's name
What to enter: Your mother's full name, split as first, middle, and last name. This field is optional unless you want your mother's name printed on the card.
Expert tip: The form asks you to select one parent whose name will appear on your PAN card. Tick your choice clearly. If you tick nothing, the father's name is printed by default.
Date of birth
What to enter: Your date of birth in DDMMYYYY format, one digit per box.
Example: Born on 2 August 1975? Write it as 02 08 1975. Notice the leading zero on single-digit days and months.
Common mistake: Writing the date in the wrong order (month first) or skipping the leading zero.
Expert tip: Your date of birth must match your proof of date of birth exactly — birth certificate, passport, or school leaving certificate. A mismatch here is a frequent rejection trigger.
Address
What to enter: Your complete residential address, including area, city, state, and PIN code. Tick the address type option that applies.
Common mistake: Leaving the PIN code blank or writing an incomplete address. For a foreign address, people often forget the country name and ZIP code, both of which are mandatory.
Expert tip: Write clearly and stay inside the boxes. This is the address your PAN card is dispatched to, so an error here can mean a lost card.
Communication address
What to enter: The address where you want your PAN card and all correspondence delivered. If it's the same as your residential address, you still need to confirm the option on the form.
Expert tip: If you're staying somewhere temporarily — a hostel, a rented flat, a work posting — use the address where you'll actually be when the card arrives. The physical card takes 15–20 working days.
Email ID
What to enter: A valid, active email address in the contact details section.
Common mistake: A typo in the email. One wrong letter and you'll never receive your acknowledgement or status updates.
Expert tip: Double-check the email character by character. This is your main channel for the acknowledgement number and every update that follows.
Mobile number
What to enter: Your 10-digit mobile number, prefixed with the country code. For India, the code is 91.
Example: Mobile number 9102511111 is written with "91" as the country code, followed by the number.
Expert tip: Use your Aadhaar-linked mobile number if you can. The online route sends an OTP here for e-KYC verification, and a mismatched number stalls the whole process.
Aadhaar number
What to enter: Your full 12-digit Aadhaar number. Quoting Aadhaar is mandatory for everyone who has one (except a small exempt category).
Common mistake: A single wrong digit, or an Aadhaar whose name doesn't match the name you've written on the form.
Expert tip: Cross-check your Aadhaar number twice. Then confirm the name on your Aadhaar matches the name field letter for letter. This one check prevents a large share of rejections.
The correction tick box
What to enter: In the left margin, tick the box beside only the fields you are correcting. Leave every other field's box untouched.
Common mistake: This is the single biggest form-filling error. People fill the whole form but forget to tick the margin box — so the system doesn't know what to change. Or they tick boxes for fields they didn't mean to touch.
Expert tip: Correcting only your name? Tick only the name box. Fixing your name and date of birth? Tick both, and leave the rest blank. The tick is the instruction — without it, nothing changes.
Photograph
What to enter: Two recent passport-size colour photographs (4.5 cm × 3.5 cm) with a white background, pasted in the boxes provided. This applies to CR-01 (individuals).
Common mistake: Stapling or clipping the photos instead of pasting them. Also, using an old or blurry photo — the print quality on your PAN card depends directly on the photo you affix.

Signature (or left-hand thumb impression)
What to enter: Your signature in two places. First, across the photograph on the left side, so part of the signature is on the photo and part on the form. Second, inside the signature box at the bottom right of the form.
Common mistake: Signing outside the box, or letting the signature cross the printed border lines. A signature that spills over the lines can get the form rejected.
Expert tip: If you use a thumb impression instead of a signature, it must be attested by a Magistrate, Notary Public, Oath Commissioner, or Gazetted Officer with an official seal and stamp. Keep your signature consistent with the one in your other official records to avoid a signature mismatch.

Signature position
Two signatures are needed on CR-01. One goes across the left-side photo, so it touches both the photo and the form. The other goes inside the box at the bottom right. The right-side photo stays clean, with no signature on it.

How to avoid rejection

Name mismatch with Aadhaar. This is the number one reason. Your name on the form must match your Aadhaar exactly. Copy it directly from your Aadhaar card.
Missing tick mark
If you don't tick the margin box beside the field you're correcting, the request is incomplete. Always tick the exact fields you're changing.
Wrong or unclear documents

Missing date of birth proof
For individuals, a separate proof of date of birth is now mandatory. Attach a birth certificate, passport, or school leaving certificate.
Signature problems

Using an old form
Forms from before April 2026 are no longer accepted. Download the current CR-01 or CR-02 only.
Blue ink or unclear writing
Use black ink and block letters so every entry is machine-readable.
